Works alongside or instead of a Living Will.

Do Not Resuscitate (DNR) / Do Not Intubate (DNI) Orders

Often separate forms, sometimes incorporated into the Living Will.

Clearly state your wishes regarding CPR or intubation.

POLST / MOLST (Physician or Medical Orders for Life-Sustaining Treatment)

Not available in all states.

Completed with a physician and used for patients with serious or terminal illness.

More detailed than a Living Will, covering day-to-day care decisions.
